UTILIZATION MANAGEMENT COMPLEX CASE MANAGEMENT NURSE

Responsible for coordinating resources, researching appropriate cost-effective alternatives/options for the catastrophic, chronically ill, identified high-risk members on a case by case basis to achieve realistic outcomes.  Optimizes member care in the ambulatory care setting to prevent/decrease emergency room visits and hospital admissions.  Works closely with members regarding their identified condition(s) and provides appropriate education directed toward member self-management.

Requires minimum 3 years high-risk/complex case management experience in an acute care setting.  Must have an active California nurse license.  Solid knowledge of Medicare, Medi-Cal, DHS, MRMIB, DMHC and NCQA regulations.  Must possess strong oral and written communication skills.  Must be customer service oriented and able to effectively interact with members, physicians and service providers.  Computer proficiency with MS Office software suite.  Must have valid California driver's license and auto insurance, and able to provide own transportation.
